New This Year—Confirmation of your registration along with your receipt/invoice and other important information will be sent to you via e-mail. Registrations without e-mail addresses will be confirmed by mail.
Corporate or Agency Team Registration Discount
Register four or more people from your company or agency at the same time and you are eligible for a team discount. Team registrations must be submitted together by fax or mail. No phone or web team registrations will be accepted. Each team member must complete a separate registration form. Teams cannot include already-discounted registrants or exhibitors. ACHMM chapters are not eligible for the team discount, but chapters may apply directly to the ACHMM to request a waiver of the conference registration fee for one delegate.
Chapter Delegates
ACHMM policy provides for a waiver of the conference registration fee for one chapter member! Only ACHMM members in good standing are eligible for this benefit. Download more information here.
If you will be a Chapter Delegate please download, print out and sign the Chapter Delegate Waiver and, if you are not a Chapter officer, have the Chapter president sign it, then submit it with your registration by mail or fax. Delegate registrations received without the delegate waiver form will not be processed. Delegate waivers cannot be applied to exhibitor registrations.

Privacy Policy
KU Continuing Education and ACHMM do not share, sell, or rent their mailing lists. You have our assurance that any information you provide will be held in confidence by KU Continuing Education and ACHMM. We occasionally use mailing lists that we have leased. If you receive unwanted communication from KUCE and ACHMM, it is because your name appears on a list we have acquired from another source. In this case, please accept our apologies.
If you do not want your registration information shared on a conference roster, please check the box on the registration form.
Program Accessibility
We accommodate persons with disabilities. Please call 785-864-5823 or mark the space on the registration form and a KU Continuing Education representative will contact you to discuss your needs. To ensure accommodation, please register at least two weeks before the start of the conference.

Refund and Cancellation policy
A full refund of preconference programs and conference registration fees will be available if requested in writing and received by August 15, 2008. After that date, refunds will be made, but a $50 administrative fee will be charged. No refunds will be made after August 29, 2008. A $30 fee also will be charged for returned checks. (please note that if you don’t cancel by the deadline and don’t attend, you are still responsible for payment.) Please send a substitute if you are unable to cancel by the deadline. No administrative fee will be charged if you provide a substitute.
A full refund of tour and special event fees will be available if requested in writing and received by August 29, 2008. The liability of University of Kansas and the Academy of Certified Hazardous Materials Managers is limited to the registration fee. KUCE and ACHMM will not be responsible for any losses incurred by registrants, including but not limited to airline cancellation charges or hotel deposits. KU and ACHMM reserve the right to cancel any program or tour if insufficient registrations are received.
